1. I agree completely ... I web form would be ideal.
2. privacy related, page 2, and most of the last page (married/divorced/single??  current and previous nationalities???) is useless, why would you want to know that type of personal info???
   
3. the pdf could also be made to include forms, since you have to print, fill-out with a pen and then re-scan or fax?
4. using Microsoft Word in a survey about open-source/free software seems a bit insulting, also see  http://www.gnu.org/philosophy/no-word-attachments.html
