This is "An Advanced Learner's Arabic-English Dictionary" by H. Anthony Salmone. It is a good-sized 19th century dictionary. Unfortunately it uses a specialized code to indicate noun and verb forms, and the encoders didn't include the code. You need to find a copy somewhere. If you have access to a good research library you have a good chance of finding it there; otherwise google a bit.
